I find that doing the meat grinding first, then cooking with water and
then blending works well.  I include the ground egg shells during
cooking, cook only long enough to get rid of all the pink, then add the
kibble after turning off the heat.  I let it cool a bit, add the other
ingredients, and blend in batches.
 
I use a hand crank grinder I bought quite cheaply on ebay.  Works quite
well and is good for building up arm muscles. :-)
