I think I can assume that by "fixed" you mean spayed.  The obvious
indication of being in heat is the swollen vulva.  A spayed female cannot
come into heat, but there are two things that will give the swollenvulva
sign.  First is a piece of an ovarie was missed when she was spayed.  The
other is adrenal gland disease.  Usually with adrenals, you also get some
balding.  In either case it requires surgery, either to remove the ovarian
remnant or to remove the diseased adrenal gland.
