Need info on incorporating as non-profit shelter

I have been put in charge of incorporating our Working Dog Rescue as a
Non-Profit.  Has anyone done this in AZ or elsewhere?
 
I'ld really appreciate it if anyone had some of the paperwork like Articles
of Incorporation, Mission Statement etc. that I could look at.
Unfortunately the books just don't give me enough information.
