I would say yes, the vulva can swell first and be the only symptom for a
while.  I had Bonnie operated on for adrenal on 5 Mar.  I had taken her
to her vet because her vulva was swollen and had a discharge.  Her vet
noticed some hair was missing on her rump.  It was a left adrenal and she
has recovered just fine.
