I called Dosh and Associates as soon as Ela Heyn told me about Heather's
post.  Sidney Dosh has been out of town a lot lately and is way behind on
his e-mail.  He is hoping to catch up this weekend, so Heather may receive
another copy of the form from him in a few days.

the inconvenience, Heather!  And anyone else who has not gotten through to
