I was under the impression that they put a dot on the right if it was a boy
and on the left if it was a girl (or vice verse) when they were born.  Then,
when they're neutered, they add another dot.
 
I had a ferret who I thought only had one dot, but one day when I was
examining the ears very closely for ear mites, I saw the second dot. It
was hidden in a brown patch of fur and not easily seen.
