Add Continental Airlines to your list.  True, they fly in the luggage
compartment, but they are first off and last on.  There is a special pet
section of the cargo that handles the pets.
 
Delta if not flown with the owner in the cabin, is flown in the luggage,
but the animal is not listed on the manifest.  Delta has also told me on
the phone that they have had animals die with them.  Even if flown Delta
Dash.  they do not track the animal.  Ask the shelter Mom of the Dook Nook
about how Delta lied to her.
