I apologize if this information has been stated in the recent past,
but I do not recall seeing a post of this data.
 
In response to the numerous reports across the FML about "near death"
reactions to FERVAC-D, I'd like to offer the following information
which appeared in the May/June 1995 issue of the AFA's American Ferret
Report.
 
Basicaly, the article stated that adverse reactions to FERVAC-D, as
well as any other drug administered to your pet(s), should be reported
to the manufacturer of the drug and the Food & Drug Administration.
 
FERVAC-D is manufactured by:       United Vaccines, Inc.

                                     1-800-283-6465
 
 
The Food & Drug Administration (FDA) has a form (FDA 1932a (2/94))
which they would like completed and returned to:

                                        Rockville, Maryland  20855-9921
 
 
Also, adverse reactions may be reported to the following, as well:

Lastly, I have permission to copy the form FDA 1932a (2/94).  I may try
to scan the form and put it in a postscript format.  That way, if anyone
needs a copy and can print from a postscript file, they could request it
from me directly.  Better yet, if BIG can set up something to automatically
shoot the file back to a requestor when they post "fda_form" to the
listserver.  What do you think, Bill?
 
›Moderator's note: Sure, we can do that, though postscript will not often
survive the email trip, so one might have to further request it be sent in
mime or uuencode format (which the server can do) but my point is, it's
not as easy as it looks.  BIG!
