Hello Weasel lovers,
 
I've been thinking of designing a database to track the health of my
four-legged family.  I was thinking that it might be good to do so that I
could extract any sort of data that might be pertinent.  I have 3 cats and
2 ferrets, and I'm forever wondering who needs what and it seems that every
time I get everyone caught up, somebody needs something else.  Also, I
think this would be good to have for emergencies and for if something
should happen to ME and someone else must step in and take care of my pets.
 
So, I am going to do a database in Microsoft Access and I'd like your help.
Please email me any ideas for fields to include.  Once I get the thing
done, I'll post it to the FML and/or to a website and people can download
it and use it.
 
So far here's what I've thought of:
 
Name
 
Species
 
Breed
 
Date of Birth
 
Immunizations by type and date
 
Surgical history
 
visits to the vet by date and reason for visit
 
diseases and ailments
 
Vet history and info (I've moved some and different vets have treated my
kids)
 
known allergies and conditions
 
Food, drink, and other daily information (in case they need to be boarded
or hospitalized, or God forbid, in case I'm boarded or hospitalized)
 
Emergency information and contacts
 
What to do with them in case of my hospitalization or demise. (Next of kin,
etc.)
 
Funeral plans for my pets
 
I'd appreciate any insight you might have on this.  I always forget things
that might be useful, so please feel free to send comments and suggestions.
