Severe ear infections like you are describing are posiible, but they
are usually secondary to ear mite infections.  Treat for ear mites
(Revolution, ivomec, milbe mite, etc), and treat the ear infection
with ear drops (Baytril otic) and Baytril orally.  Of course treat the
dehydration with SQ or IV fluids.  Feed a high protein food (baby food,
a/d, carnivore care) in addition to their regular food.
