vaccine reaction - contacting USDA

Looking through the searchable archives, I saw suggestions to let both
the manufacturer *and the USDA* know if an animal has had a reaction to a
vaccine.  I want to ask my veterinarian to do this (I posted a few days
ago about my ferret's reaction to his distemper vac.)
 
Can you let me know address or fax #, etc, at USDA to send this
information?
