Hello all.  I'm coming out of my anonymous hole.  I posted about the SPCA
screening process for the adoption of ferrets.  I work at the local SPCA in
the Cruelty Investigation and Animal Rescue Dept.  The ferret is the most
neglected animal at the SPCA and the process is horrible.  As a fuzzy
friend , I've been asked to produce a form or forms that is appropriate
for screening potential adopters.  The help I need from all of you is
ideas in the form of a copy of the forms that you use to screen potential
adopters or the specific questions that you ask in verbal interviews.
Please help........
