It's not a vaccination, it's a monthly, oral medication.  I suppose you
could return to your vet repeatedly for injections to prevent heartworms.
I've never heard of anyone going that route and it's not necessary when
there are so many ways to treat them orally and at home.
 
After they're tested to be sure they are heartworm negative, they can be
put on any one of a number of different heartworm preventatives.
Personally, I use Interceptor for 2-10 pound dogs.  One tablet is just
right for mine since they all weigh two pounds or more.  Some use
Heartguard for cats.  Talk to your vet, he or she should know what to use.
