passed out annually at the HSUS convention and all year long by STAR* at
other events or when requested - not to mention put on the WWW and in
several computer service libraries), I need you to fill out the annual STAR*
shelter survey and send it back to me!

If you want to be listed on STAR*'s Resource Listing, you MUST return this
form.  The database needs culling and this is the one way to assure real
resources are listed.  Thank you for your time.

22151-0714.  Enclose a SASE (#10 - the long one) with one stamp for each
list and specify if you would like:
 
1) results of the survey [out February],
2) a copy of the final resource listing [out February] or
3) a copy of just your state's resources [vets, clubs, shelters, breeders,
   etc.- anytime].
 
It would be appreciated if this survey could be returned before January 15,
1996.
