    Sure sounds like adrenal disease to me.  I think based on the clinical
signs, I would go for the surgery and skip the ultrasound.  The ultrasound
won't do much - probably will just suggest surgery anyway.  If the vet
thought she was in heat and gave antibiotics, that doesn't make much sense.
Spayed females don't come into heat, and antibiotics don't do anything for
animals in heat anyway.
 
    I have dealt with a pretty good surgeon in Seattle - - in Lynnwood.
His name is Steven Stoll at the Seattle Vet Hospital for Surgery - I'm sure
he can do a good job for you.  (206-743-5802).
