Your best bet is to contact the Ministry of Health for Ontario.
Try the General Inquiry number.  The number here is
1-800-268-1153.  Which may or may not work from the US.  If the
1-800 number doesn't pan out, try directory information for
Toronto, Ontario, Canada.  I believe you want the Infectious
Diseases Branch which would either be in Toronto, or Kingston.
[Toronto is the capital of Ontario, and most stuff was there (IDB
was in Toronto back in '78), but MOH has gone through some
decentralization in the past couple of years, so the IDB may have
moved - probably to Kingston.]
