Another possibility may be a yeast infection.  Malassezia pachydermatis
generally does not respond to Baytril and may actually benefit from the
moist environment imparted by the presence of drops twice daily.
 
Another swab should identify eggs or mites if they are the problem, and if
the vet is good with a microscope (some are, some aren't), the yeasts are
fairly characteristic in appearance.
